Whether you just want to notify us of a change of address, sign up for direct deposit or even file an application for your pension, there is a form for each situation!
2 ways to do it!
1. Use our online services
For certain forms, applications can be filed online. You can click
Apply online in the
list of forms or consult the list of our
online services.
2. Use our PDF forms
Important!
You must complete the PDF forms from a desktop or laptop computer. The Adobe Acrobat Reader application does not allow you to complete the fields of our protected documents on a tablet or mobile phone.
- Download a copy of the form in PDF format to your computer.
- Open
Adobe Acrobat Reader , then the form you downloaded to your computer to complete it onscreen.
- Save the completed form on your computer.
Make sure that your information has been saved in your PDF file.
- If necessary, print the document and sign it. An
electronic signature is also accepted for certain forms.

Send the form online or as indicated on the form.

list of forms and find the following note:

An electronic signature is accepted on this form.
You can add your signature with one of the following electronic processes:
- signature on a touchscreen using your mouse, stylus or finger
- scanned handwritten signature (image of your signature).
Important: Typed signatures are not accepted.
