Application for the Supplement for Handicapped Children

You are entitled to this financial assistance if:

  • you are receiving Family Allowance for a child concerned by the application for the Supplement for Handicapped Children.
  • the child is under age 18 and has a physical impairment or a mental function disability. The child's condition significantly limits him or her in carrying out his or her life habits compared with what is expected for a child his or her age. The expected duration of the disabilities must be for at least one year. We determine your child's eligibility for the Supplement for Handicapped Children based on certain requirements.

If you do not receive Family Allowance, you can send your Application for Family Allowance Payments and your Application for the Supplement for Handicapped Children at the same time.

The form the Application for the Supplement for Handicapped Children includes:

  • the application for the Supplement for Handicapped Children:
    • the Parent's part, which must be completed by the parent;
    • the Professional's part, which must be completed by a health professional (physician, physiotherapist, occupational therapist, etc.) who knows the child's condition the best. This person must be a member of a professional order and have assessed or seen the child again in the last 12 months.
  • the Educational Achievement Report (School or Daycare);
  • the Consent Regarding the Release of Medical, Psychosocial and Education Related Information.
