Aplication for the Supplement for Handicapped Children

Eligibility

You have a dependent child under age 18 who has a physical impairment or a mental function disability. The child's conditionsignificantly limits him or her in carrying out his of her life habits compared with what is expected for a child his or her age.
The expected duration of the disabilities must be for at least one year.

Application for the Supplement for Handicapped Children Requiring Exceptional Care

Eligibility

You have a dependent child under age 18 who has severe and multiple disabilities and/or or whose medical condition requires complex medical care. The child's condition must meet the eligibility requirements for a period expected to last at least one year.

Educational Report
(School or Daycare)
To assess the child's functioning in daycare or at school for an application for the Supplement for Handicapped ChildrenThe form is included with the application for the Supplement for Handicapped Children.
Functional-Independence Report
(School or Daycare)
To assess the child's functioning in daycare or at school for an application for the Supplement for Handicapped Children Requiring Exceptional CareThe form does not need to be sent right away with the application for the Supplement for Handicapped Children Requiring Exceptional Care. We will directly contact the school or daycare to obtain it, if it is required.
Consent Regarding the Release of Medical, Psychosocial and Education-Related Information
The form is included with the application for the Supplement for Handicapped Children and the Supplement for Handicapped Children Requiring Exceptional Care.
